Parent Disclosure Statement


The California Health and Safety Code, Section 120440, authorizes the local health officers to operate an immunization information system (registry) and allows authorized health care providers and other approved agencies to exchange immunization information electronically unless a patient declines participation in the system.

The Disclosure Statement is intended to inform the client/patient of their rights. Additional copies may be requested from the Immunization Program.
