CHDP Clinics


Program Focus:

To provide well-child exams and immunizations to Medi-Cal recipients from birth to age 21 and non Medi-Cal eligible children and youth from birth to age 19 whose family income is low to moderate.

 

Program Activities:

We are a CHDP provider who offers a full range of health assessment services including:

  • Health and developmental history
  • Physical examination
  • Nutritional assessment
  • Immunizations
  • Vision testing
  • Hearing testing
  • Tuberculin skin testing
  • Laboratory test
  • Health education and anticipatory guidance
  • Tobacco Education
  • WIC Referral for children up to age 5
