Case Management and Eligibility


Case Management:

California Children’s Services (CCS) staff at provide case management services. Case management determines medical eligibility, deciding what your child needs, and helping get special doctors and services to meet the child’s needs. This includes assistance in the coordination of medical care and referrals to medical specialists; follow-up with other care providers such as schools, public health or school nurses, social workers and other agencies; transferring medical records; locating new facilities; and services when families move; and assisting the family with coping issues. Every CCS client receives case management.

Eligibility:

Individuals neet to meet the following requirements in order to be eligible:
Be a resident of California, be under 21 years of age, have or may have a medical problem that CCS covers and have an income under $40,000 a year. Individulas can still get CCS services if their income is over $40,000 if you they Medi-Cal, Healthy Families, or their out of pocket medical expenses for their child’s health care is more that 20% of the family's income. Clients that only want MTU services are also able to access CCS services.

PROGRAM LIMITATIONS:
CCS does not cover all situations. CCS covers most problems that are physically disabling or that need to be treated with medicines, surgery, or rehabilitation. CCS covers children with problems like the following:





Congenital heart disease

Cancer, tumors

Hemophilia, sickle cell anemia

Thyroid problems, diabetes

Serious chronic kidney problems

Liver or intestinal diseases

Cleft lip/palate, spina bifida

Hearing loss, cataracts

Cerebral palsy, uncontrolled seizures

Rheumatoid arthritis, muscular dystrophy

AIDS

Severe head, brain, or spinal cord injuries

Severe burns

Problems caused by premature birth

Severely crooked teeth

Broken bones
