Program Focus:

 

The Healthy Families Program offers low cost insurance for children and teens

up to age 19.  It provides health, dental and vision coverage to children who

meet the program rules and do not qualify for free Medi-Cal.

 

People who qualify for Healthy Families get 12months of health coverage, as long as they pay the

monthly premiums.  They get health services through the health, dental

and vision plans that work with Healthy Families.  Each member can choose his or her own plan. 

?         Healthy Families members pay a low monthly premium.

?         The Healthy Families plans cover most visits to doctors, dentists and

specialists, and prescriptions.

?         The Healthy Families plans cover the costs of hospital care, labs and

preventive care. 

?         Each county in California has different plans to choose from.

 

Imperial County Public Health Department staff is available to assist the public with the Healthy Families

application process.  Bilingual Staff is available for those who are limited English speaking.  No appointment

is necessary to get assistance with the Healthy Families Application.  

 

Please note that the Imperial County Public Health Department is open Monday through Friday (except Holidays)

from 8:00 am to 12 noon and 1:00 pm to 5:00pm.    

 

 

Documents needed to apply for Medi-Cal or Healthy Families

 

Proof of income

 

Bring a copy of your most recent paycheck stub.  If your spouse works, be sure to also bring a copy of his/her most recent paycheck stub. You may bring more than one paycheck stub if you get paid more than once a month and the amounts differ from paycheck to paycheck.

 

OR bring a copy of last year?s Income Tax Documents

 

OR a letter from an employer/s that states how much money you or your spouse makes and how often you or your spouse is paid.

 

Birth certificates or proof of immigration status

 

Bring copies of birth certificates or proof of immigration status for the children you are applying for.

 

Social security number

 

Bring the social security number or the children applying for Medi-Cal.

 

Proof of deductions or expenses


Bring proof of deduction or expenses for the children and/or dependent care.

 

Proof of any child support or alimony

 

Bring proof of any child support or alimony you receive or pay.

 

Proof of California residency

 

Bring proof of residency.

 

 If you are unable to bring copies of any of the above documents, we might still be able to help you.
