Vaccines and Immunizations
A: You may visit the Imperial County Public Health Department to request a copy of your immunization record. You will need to provide valid identification and pay the required $5 fee.
If you are unable to come in person, you may complete and sign the Authorization for Release of Protected Health Information form and submit it to the department by mail or fax.
If you are requesting an immunization record on behalf of someone who is 18 years of age or older, you must also submit an Authorization for Release of Protected Health Information form.
Digital immunization records are also available through the California Department of Public Health.

A: Walk-ins are welcome; however, appointments are encouraged. To schedule an appointment, call 442-265-1444 or schedule below.
Please note: Clients needing a TB test for employment or travel immunizations must call the Health Clinic at 442-265-1420 to schedule an appointment.

Requesting Vital Records
Birth Certificates
A: The Imperial County Public Health Department issues birth certificates if the birth occurred in Imperial County within the last two years.
If the birth occurred more than two years ago, the request must be made through the Imperial County Recorder’s Office.
To request a birth certificate from the Imperial County Public Health Department:
- Complete a Birth Certificate Application. The application is available at the department or online below:
Be an authorized requestor. Please refer to the application for information on who is eligible to request a certified copy.
Present a valid government issued photo ID.
Pay the required fee of $31 per certificate.
Completed applications must be submitted in person or by mail. Birth certificates cannot be requested entirely online.
If submitting by mail, the application must include a notarized sworn statement of identity, as required by state law. Please include a check or money order made payable to Imperial County Public Health Department for the appropriate fee of $31 per certificate requested.
A: If the certificate has been registered, in-person requests may be processed and issued the same day. Requests submitted by mail are processed the same day they are received and mailed the next business day.
A: Yes. The fee is $31 per birth certificate.
Death Certificates
A: The Imperial County Public Health Department issues death certificates if the death occurred in Imperial County during the current year or the previous year.
If the death occurred before that time period, the request must be made through the Imperial County Recorder’s Office.
To request a death certificate from the Imperial County Public Health Department:
Complete a Death Certificate Application. The application is available online below:
Be an authorized requestor. Please refer to the application for information on who is eligible to request a certified copy.

Pay the required fee of $26 per certificate.

Medical Records
A: You may visit the Imperial County Public Health Department to request a copy of your medical or health records for services received through the department.
If you are unable to come in person, you may complete and sign the Authorization to Receive Protected Health Information form and submit it to the department by mail or fax. Please include a copy of a valid government-issued photo ID.

Requests for copies of, or access to, medical records will be processed within 30 days of receiving the request.
A: There is no fee for medical records.
However, if you are requesting immunization records, there is a $5 fee for a duplicate immunization record from the California Immunization Registry.
Volunteer Opportunities
A: Individuals interested in volunteering with the Imperial County Public Health Department outside of an academic program may contact the Workforce Development team to inquire about current volunteer opportunities.

- Desired volunteer timeframe (number of weeks or months)
- Availability (specific days, times, and number of hours available per week)
Eligibility Requirement:
- Must be at least 18 years of age.
This information helps ICPHD determine whether a volunteer opportunity is available that aligns with the individual's interests, qualifications, and the department's current needs. If a volunteer opportunity is available, applicants will be contacted by the Workforce Development team and guided through the County's volunteer onboarding process.
A: Students interested in completing an internship for academic credit with ICPHD should first check with their college or university to determine whether a current agreement with the department is in place, as an agreement is required before an internship placement can be made.
If an agreement is in place, students may contact the Workforce Development team and provide:
- Resume
- Academic program or field of study
- Areas of interest
- Required internship hours
- Preferred internship timeframe and availability
This information helps ICPHD determine whether an internship opportunity is available that aligns with the student's academic requirements and current Department needs.
If an agreement is not currently in place, ICPHD may work with the educational institution to explore establishing a partnership.

Animal Adoption and Reporting

A: No. The Imperial County Public Health Department does not provide lice treatment.

A: No. Bed bugs have not been shown to transmit diseases to humans.
